Provision of a permit for sanatorium treatment to the pregnant spouse of a participant of the SVO

Applicant:

• the pregnant spouse of a participant in a special military operation in the territories of Ukraine, Donetsk People's Republic, Luhansk People's Republic, Zaporozhye and Kherson regions, residing in the Lipetsk region, her representative.

The following documents are provided:

• the identity document of the applicant, or the identity document of the representative, and the document confirming the powers of the representative (if the representative applies);
• a certificate from a medical organization for obtaining a permit for sanatorium treatment in a form approved by the Ministry of Health of the Russian Federation.

It is necessary to contact:

• to the multifunctional center, to the social curator.

The maximum period of consideration of the application:

• 10 working days.

Provision of sanatorium-resort vouchers:

• within 10 working days following the day of the decision.

The arrival date is agreed with the applicant by an employee of the Department of Social Policy of the region after making a decision on granting a permit.

The duration of arrival is 24 days, treatment, three meals a day, accommodation are included in the price
