Development of oncological services in the Lipetsk region

Every year, on February 4, the World Cancer Day is celebrated in all countries, the purpose of which is to remind about the danger of this disease. This date is also relevant for our country, where cancer annually claims about 300 thousand human lives.

The task of preventive measures is to draw public attention to the global problem of oncological diseases, as well as to remind how dangerous and widespread these diseases are now.

Currently, a multi-level system of providing oncological care to the population has been formed and is effectively functioning in the region, providing timely detection and treatment of neoplasms of this pathology.

The oncological service of the Lipetsk region is represented by the regional specialized institution of the Lipetsk Regional Oncological Dispensary, which includes: 6 surgical departments, 2 chemotherapy departments, a radiological department, a palliative care department, a day hospital.

Four centers of outpatient oncological care (CAOP) are actively working, created in order to reduce the time of diagnosis and improve its quality on the basis of: GUZ "Lipetsk City Hospital No. 3 Svobodny Sokol", GUZ "Yeletskaya City Hospital No. 2", GUZ "Yeletskaya City Hospital No. 1 named after N.A. Semashko" and GUZ "Lipetsk City Hospital No. 4 "Lipetsk-med". All of them have a full range of necessary equipment and specialists for the timely diagnosis of malignant neoplasms.

In addition, 33 primary oncological offices and 41 examination rooms are engaged in early diagnosis in the region, deployed on the basis of healthcare institutions in cities and districts of the region.

Improving the timely diagnosis of malignant neoplasms, equipping medical equipment for highly effective treatment of tumors and the introduction of advanced treatment methods are the main directions of development of the cancer service of the Lipetsk region.

Within the framework of the regional project "Fight against oncological diseases" on the re-equipment of medical equipment:

- in 2019, 24 units of medical equipment worth more than 200 million rubles were purchased, including a mammograph, a multispiral computed tomograph, a close-focus radiotherapy device, an X-ray diagnostic complex for 3 workstations, an ultrasound system for a video gastroscope, a videobronchoscope with adaptive frame interpolation and autofluorescence image functions, a video endoscopic complex, - all equipment was put into operation.

- in 2020, 36 units of medical equipment worth more than 372.6 rubles were purchased,

- in 2021, for the purpose of re-equipment, 6 units of medical equipment were purchased with a total cost of 135.9 million rubles:

- combined combined system of single-photon emission computed tomography and computed tomography - 74.9 million rubles;

- brachytherapy apparatus - 56.3 million rubles;

- electrosurgical unit - 2 pcs. 4.1 million rubles;

- microscope - 2 pcs. 593.8 thousand rubles.

A single-photon emission computed tomograph combined with an X-ray computed tomograph (SPECT/CT) allows:

- to carry out timely diagnostics of oncological diseases with determination of localization and prevalence of the process,

- to differentiate malignant and benign formations of various organs and systems (cardiovascular, endocrine, urinary systems, musculoskeletal system, digestive organs, lungs),

- to identify the presence of structural changes and functional disorders at the stage of minimal clinical manifestations of the disease.

The use of hybrid technology significantly reduces the path to the correct diagnosis, the choice of optimal treatment tactics with minimal radiation loads, time and financial costs. The use of the acquired brachytherapy device will reduce the number of complications in patients in comparison with remote radiotherapy and surgical interventions, reduce the length of stay in the hospital and the rehabilitation period.

Currently, radiation therapy, along with medicinal and surgical, is the most important, and in some cases the only method of treating cancer and is used both for radical and palliative purposes. To date, more than 70% of cancer patients need radiation treatment. About 2000 people are treated annually at the radiological center of GUZ "LOOD".

The introduction of a new 4-storey surgical building with nine modern technological operating rooms and a department of anesthesiology and intensive care will significantly increase the availability of high-quality high-tech medical care for patients with malignant neoplasms. 940 pieces of equipment have already been purchased for the new operating unit with the intensive care unit, including: expert-class ultrasound machines, an ultrasonic harmonic scalpel, an endovideoscopic complex for performing abdominal, thoracic, urological, gynecological and ENT operations, an angiographic installation, an operating microscope, a laser surgical complex, mobile X-ray machines, surgical tables, sterilizers of instruments, medical furniture.

It should be noted that the technology of "lean manufacturing" has been introduced in the GUZ "LOOD", the operation of the operating unit, the patient routing system, and the support of low-mobility patients have been optimized. The polyclinic has a call center, an emergency patient admission system.

Telemedicine technologies are actively used. So, in 2020, about 417 telemedicine consultations were held in national medical research centers and more than 95 telemedicine consultations with medical organizations of the Lipetsk region.

The ONCONET telemedicine platform, an interactive remote patient monitoring service with an accessible information site, continues to develop to support cancer patients during long-term treatment after discharge from the hospital.

Today, 128 doctors work in the Lipetsk Regional Oncological Dispensary. Among them are 2 Doctors of Medical Sciences, 6 candidates of Medical Sciences, 1 "Honored Doctor of the Russian Federation", 3 "Honored Health Workers of the Lipetsk region". One doctor was thanked by the President of the Russian Federation.
