You can get a certificate of vaccination at the MFC and medical organizations with equipped workplaces of MFC employees.
From November 16, 2021, in all medical organizations of the region, residents of the region can receive a certificate of preventive vaccinations against a new coronavirus infection (COVID-19) or the presence of medical contraindications to vaccination of a form approved by the Ministry of Health of Russia without a QR code.
The certificate is issued on the basis of an oral request from a citizen immediately after each case of the introduction of the vaccine (its components)/the establishment of a medical contraindication to vaccination or at another time of treatment, is certified by the signature of the doctor indicating his surname and initials of the name and patronymic (if available) and the seal of the medical organization (if available).
A medical certificate of preventive vaccinations against a new coronavirus infection (COVID-19) or medical contraindications to vaccination and (or) a disease caused by a new coronavirus infection (COVID-19) is automatically generated in Russian and English no later than 3 calendar days after being entered into the information resource of information accounting (federal registers), contains a two-dimensional barcode (QR code).
When updated information about vaccinations, medical contraindications and (or a previous illness appears in the information resource (federal registers), the medical certificate is reissued with the information previously entered in it.
If a citizen has a confirmed account on the EPSU, he can contact the MFC or an employee of the MFC, whose workplace is organized in a medical organization (if available), which will provide him with the opportunity to access his personal account on the EPSU and print a medical certificate.
In the absence of information about the certificate generated automatically on the EPSU, the MFC employee informs the citizen about the possibility of contacting or sending a complaint to medical organizations at the place of residence (attachment, vaccination, treatment for a new coronavirus infection (COVID-19)) in writing, electronically, as well as through the EPSU.
